The POLST (Portable Orders for Life Sustaining Treatment) Education & Access Fund supports efforts to expand awareness, understanding, and access to POLST, helping individuals communicate their goals and preferences for care. Contributions support education and public engagement designed to encourage thoughtful conversations and greater clarity about care for people facing serious and life-threatening illness. The fund also supports ongoing efforts to strengthen and advance POLST as a resource for individuals, families, and healthcare professionals.
